Working with Referral Sources for a Mobility Evaluation
Wheelchairs Plus has worked with and referred to these facilities and professionals in the past. Please consider their strengths when choosing an organization or person to do your Mobility Evaluation.  Many insurances have qualifying criteria that make it important to honestly and completely state all of your limitations and strengths. This should include your diagnosis, physical conditions, and pain experienced.  Help the professionals you work with to understand how the equipment will allow you to do things you can not do now.  Consider how and where you will use the equipment and the performance you expect.  All of these facts will help the professionals to clearly state your needs for insurance purposes.  To help determine how the equipment will work for you, call Wheelchairs Plus.
 
Facilites

These can include a nursing home, hospital or therapy center that has trained personnel to do mobility evaluations.   The facility will support the professionals with the required training to meet insurance needs.
Professionals

Qualified professionals such as Physical and Occupational Therapists or a Physicatrist (physical medicine doctor) will complete thorough evaluations including the following information: 
They will rule out of less costly mobility equipment such as canes, crutches, walkers. 
If you need a power chair or scooter then a manual wheelchair must be ruled out. 
The professional will document your diagnoses with symptoms, history of your condition, physical examination as it relates to the equipment, and also a functional assessment. 
Sometimes special forms must be completed and we will provide these to the person you choose when needed. 
When making an appointment always tell the scheduling person you need a "Mobility Evaluation". 


Grand Itasca Rehab Facility

 

Dawn Carlson, Physical Therapist has been trained on Medicare and MN MA requirements. She has a special interest in equipment and has been doing mobiity evaluations for several years.
